The purpose of this form is for our candidates to make declarations as part of a due diligence process for employment, as well as reference contact details and terms & conditions of our agency. 

Cultivar Asia Pte Ltd will use the personal data that you have provided herein, and information that we have obtained from other sources, to assess your employment potential, statistical purposes, and for carrying out appropriate reference/background checks on behalf of our client company. By submitting your application, you consent to the collection, use and/or disclosure of your personal data to our employees and hiring company, as required, to fulfil these purposes. 

When you provide us with personal data about a third party (e.g. referees), you confirm that you have obtained the consent of such third party for the disclosure of their personal data to us, and the subsequent processing by us of their personal data.
